腰椎神经根管及鞘膜切开减压在腰椎管狭窄症术中的应用

Application of lumbar nerve root canal and sheath decompression in lumbar spinal stenosis

目的探讨腰椎管狭窄症的病因和手术中神经根管及鞘膜切开减压的方法和临床效果。方法对58例腰椎管狭窄症患者行神经根管减压及神经根鞘膜切开减压。结果术后24小时内均有不同程度下肢轻松感,肢体麻木缓解和神经根疼痛症状完全消失。39例术后2周内症状完全消失,平均治愈时间为7天。结论腰椎管狭窄症的病因主要是腰椎神经根管狭窄,在神经根减压与粘连松解同时行神经根鞘膜切开减压后能显著缓解神经压迫症状,加速神经功能恢复。

Objective To explore the cause of lumbar spinal stenosis and investigate the clinical results of using nerve root canal surgery and sheath decompression.Method Fifty eight cases of nerve root canal and sheath decompression in treating lumbar spinal stenosis were analyzed.Results Within twenty four hours after the operation,a relaxed feeling in the lower limb was achieved in all patients with the amelioration of numbness,and complete relief of nerve root pain.In thirty nine patients,the symptoms disappeared completely after two weeks and the average recovery time was seven days.Conclusion The main cause of lumbar spinal stenosis is constriction of nerve root canal.Performing nerve root canal and sheath decompression simultaneously with lumbar spinal canal decompression and adhesions lysis can significantly relieve symptoms of nerve compression,and accelerate the recovery of neurological function.
