摘要
目的 评价燃煤型慢性砷中毒患者的神经系统损害。方法 随机检测 5 1例患者的肌电图 ,运动神经传导速度 (MCV) ,感觉神经传导速度 (SCV) ,F波 ,H反射 ,体感诱发电位 (SEP)。结果 12 0块肌肉肌电图6 6 .7%以多相不规则波为主 ;MCV与 SCV均有减慢 ,以 SCV减慢最为显著 ,而 MCV远端动作电位潜伏期明显延长 ,神经传导速度 (NCV )的异常远比纤颤、正尖波检出率高 ,且随病情的加重改变愈加明显 ;F波与 H反射异常率不高 ,提示很少累及神经根 ;SEP的异常率高达 6 0 .8% ,同时说明该组患者伴有中枢神经的损害。结论 神经电生理检测可以用来观察燃煤污染型慢性砷中毒患者的神经系统损害程度、病程、范围 。
Objective In order to evaluate the neurosystemic damage in patients suffered from coal burnt arsenicalism.Methods The electromyogram,motor nerve conduction velocity(MCV),sensory nerve conduction velocity (SCV),F wave,H reflex and somatosensory evoked potentials (SEP) were randomly detected in 51 patients with coal burnt arsenicalism.Results In 66.7% of 120 muscles detected the electromyogram showed mainly polyphase irregullar waves;MCV,especially SCV was slowed down;the latency of distant MCV action potentials was prolonged obviously;the abnormality rate of nerve conduction velocity was much higher than that of fibrillation,and was more obvious with the aggravation of the illness;the low abnormality rate of F wave and H reflex suggested that the illness hardly involved the nerve roots;the high abnormality rate of 60.9% of SEP suggeted that this group of patients were companied with the damage of central nerves.Conclusions The detect of electroneurophysiology can be used to determine the extent,course and range of nerve system damage in patients with coal burnt arsenicalism and is one of the early detecting means of this disease.
