摘要
目的 观察OrbscanⅡ系统测量白内障超声乳化手术前后角膜地形图的变化趋势.方法 对40例年龄相关性白内障患者的40只眼行超声乳化吸出术,用OrbscanⅡ系统观察患者双眼手术前,手术后1d、1周、1个月、3个月时的角膜地形图的变化.结果 ①手术眼3mm及5mm区总角膜曲率在术后1d比术前减小,差异有显著性（P〈0.05）,1周、1个月、3个月时与术前相比,差异无显著性（P〉0.05）,且术后1周、术后1个月、术后3个月两两相比差异无显著性（P〉0.05）.②手术眼手术后3mm及5mm区总散光、Irreg值及角膜厚度在术后1d、1周时较术前增加,差异有显著性（P〈0.05）,术后1个月、3个月时与术前相比,差异无显著性（P〉0.05）,且术后1个月与3个月相比差异无显著性（P〉0.05）.③非手术眼总角膜曲率、总散光、Irreg值及角膜厚度在术前、术后1d、1周、1个月、3个月时两两比较差异无显著性（P〉0.05）.结论 用OrbscanⅡ系统可较全面评估角膜参数的变化、角膜曲率、角膜散光在术后1个月内基本恢复至术前水平,并保持稳定.
Objective To evaluate the change of topography by orbscan II before and after phacoemulsifi- cation. Methods phacoemulsification with age-related cataract were performed on 40 eyes of 40 patients, another 40 eyes was not. The change of topography of two eyes was analyze by Orbscan Ⅱ before and after phacoemulsification on the first week, the first month and the third month. Results To the eyes which were performed on surgery, on the first day the Chief 3mm and 5ram corneal curvature was lower than the preoperative level （ P 〈 0.05 ） , it returned to preopera- tive level on the first week, and it was stabilized at this level { P 〉 0.05 ） , and on the first week, the first month and the third month ,each of them had no significant differences （P 〉 0.05 ）. On the first day and the first week, the 3mm and 5mm corneal astigmatism, corneal irregularity and corneal thickness increased significantly （P 〈 0.05 ）, and then it gradu ally fell back to preoperative level after 1 month and keep this level until the third month （ P 〉 0.05 ）. To the eyes which were not performed on surgery, corneal curvature, corneal astigmatism and corneal irregularity had no significant differ ences （ P 〉 0.05 ）. Conclusion A comprehensive assessment of topography can get by Orbscan 11 system, corneal curva ture,corneal astigmatism,corneal irregularity and corneal thickness resume to the preoperative level after one month,and keep this level until the first month.
