影响距骨骨折手术疗效的因素分析

目的探讨影响距骨骨折手术效果的相关因素。方法选择2008年5月～2011年5月收治的距骨骨折患者26例,就其临床资料进行回顾性分析。择期手术14例,急诊手术12例。加压固定19例,克氏针及普通螺钉非加压固定7例。结果本组26例患者中,解剖复位14例,优良率为85.7%(12/14);满意复位9例中,优良率为55.6%(5/9);复位不良3例中,优良率为0,术后功能评分解剖复位优于满意复位和复位不良优良率,差异有统计学意义(P<0.05)。19例加压固定组患者中,优良率为47.4%(9/19);7例克氏针和普通螺钉组中,优良率为57.1%(4/7),差异无统计学意义(P>0.05)。术后创伤性关节炎5例,距骨坏死4例,骨折愈合不良3例。结论对影响距骨骨折手术效果的因素进行分析,选择相应的治疗方法,可提高手术成功率。

Objective To investigate the impact of the talus fracture surgery effect.Methods Talus fracture patients admitted to our hospital from May 2008 to May 2011,26 cases,their clinical data were retrospectively analyzed.Elective surgery in 14 cases,12 cases of emergency surgery.19 cases of compression fixation,the Kirschner wire and ordinary screw non-pressurized fixed seven cases.Results In 26 patients in this group,14 cases,85.7%（12/14）;satisfactory reduction in 9 cases,good rate of 55.6%（5/9）;malreduction three cases,the fine rate 0,the function score after anatomic reduction is better than the excellent rate of satisfaction reset and reset bad,the difference was statistically significant（P0.05）.19 cases of compression fixation group patients,the excellent and good rate was 47.4%（9/19）;seven cases of Kirschner wire and ordinary screw group,the excellent and good rate was 57.1%（4/7）,the difference was not statistically significant（P0.05）.Postoperative traumatic arthritis,5 cases of talar necrosis in four cases of fracture healing of three cases.Conclusion Analysis of the factors that affect the talus fracture surgery effect,select the appropriate treatment can improve the success rate of surgery.
