布拉氏酵母菌预防婴幼儿肺炎继发抗生素相关性腹泻的临床观察 被引量:11

Clinical Observation of Saccharomyces Boulardii in the Prevention of Infants Antibiotic Associated Diarrhea Secondary to Pneumonia

摘要 目的:观察布拉氏酵母菌预防婴幼儿肺炎继发抗生素相关性腹泻(AAD)的效果。方法:将300例肺炎患儿随机分为预防组与对照组,各150例。2组均给予抗生素抗感染及对症支持治疗,预防组在使用抗生素的同时口服布拉氏酵母菌,对照组在使用抗生素的同时不使用任何微生态制剂。2组患儿在出现腹泻后均给予口服蒙脱石散剂、补液等对症支持治疗。观察2组患儿继发腹泻的发病率、腹泻严重程度、腹泻持续时间、大便平均次数及抗腹泻疗效。结果:预防组发生AAD32例(占21.33%),均为轻型腹泻,无重型腹泻;对照组发生AAD70例(占46.67%),其中轻型腹泻48例(占32.00%),重型腹泻22例(占14.67%),2组AAD发病率及腹泻严重程度比较差异有统计学意义(P<0.05)。2组AAD腹泻持续时间及腹泻平均便次比较差异也有统计学意义(P<0.05)。预防组与对照组的抗腹泻总有效率分别为93.75%、64.29%(P<0.05)。结论:布拉氏酵母菌能有效降低婴幼儿肺炎继发AAD的发病率,缩短病程,减轻腹泻症状,无不良反应。 OBJECTIVE: To observe the effects of Saccharomyces boulardii on the prevention ofinfants antibiotic associated diarrhea (AAD) secondary to pneumonia. METHODS: 300 infants with pneumonia were randomly divided into prevention group and control group with 150 cases in each group. 2 groups were both given regular anti-infective treatment and symptomatic support treatment. The prevention group was additionally given Saccharomyces boulardii orally while no microbial ecological agent was administrated in control group. 2 groups were both given treatments of Smectite powder and fluid infusion after diarrhea. The incidence, symptom and duration of AAD, defecation frequency and therapeutic efficacy in two groups were compared. RESULTS: There were 32 AAD cases in prevention group (21.33%), which were mainly mild diarrhea without severe diarrhea. There was 70 AAD cases in control group (46.67%), among which 48 cases were mild diarrhea (32.00%) and 22 cases were severe diarrhea (14.67%). There was significant difference between 2 groups in the incidence and severity of diarrhea (P0.05); there was statistical significance in the diarrhea duration and average defecate number between 2 groups (P0.05); the total effective rates of prevention group and control group were 93.75% and 64.29% (P0.05). CONCLUSION: Saccharomyces boulardii is effective and valuable for decreasing the incidence ofinfants AAD secondary to pneumonia, and it shortens the duration of AAD and relieves diarrhea. No adverse reaction has been found.
