整体辨证治疗膝骨关节炎诊疗方案临床应用评价

Clinical Application Study on Diagnosis and Treatment Scheme of Knee Osteoarthritis Treated by Integral Syndrome Differentiation

目的:观察整体辨证治疗膝骨关节炎诊疗方案的临床疗效及其安全性。方法:对2007年3月—2011年3月膝骨关节炎120例行整体辨证治疗的患者进行回顾性分析,随机分为两组,治疗组患者予以中医整体辨证、多途径治疗方法,对照组病例予以非甾体类药物塞来昔布胶囊消炎止痛治疗,将两组治疗前后的证候进行总结分析,临床评价采用HSS膝关节评分进行分析,并对中医证候进行系统疗效评价。结果:治疗组治疗后膝关节疼痛、功能、活动度与治疗前比较有显著性改善(P<0.01),对照组与治疗前比较,除疼痛指标略改善外(P<0.01),余指标变化不大(P>0.05),两组间各证候比较,功能、活动度、屈曲畸形及稳定性相比有显著性差异(P<0.01),在疼痛及肌力方面无明显差异(P>0.05),治疗组优良率大于对照组优良率,有显著性差异(P<0.05)。治疗组总有效率91.67%,对照组总有效率81.67%。两组患者中医证候疗效比较有显著性差异。结论:整体辨证治疗膝骨关节炎诊疗方案安全有效,容易推广应用。

Objective：To study the therapeutic effects and safety of diagnosis and treatment scheme of knee osteoarthritis treated by integral syndrome differentiation.Methods：120 knee osteoarthritis cases treated by the whole dialectical treatment from 2007 March to 2011 March were retrospectively analyzed.All the patients were randomly divided into two groups,the patients of treatment group were treated by Chinese medical syndrome differentiation and multiple treatment methods;the patients of control group were treated by non-steroidal antiphlogistic and analgesic drug Celecoxib capsule.The therapeutic effects and HSS Knee score before and after treatment in the two groups were investigated and analyzed.Results：The knee pain,function and activity degree of the patients of treatment group were significantly improved after treatment（P〈0.01）.Compered with before treatment,the pains of patients of control group were slightly improved（P〈0.01）.Other indexes were little changed（P〉0.05）.Function,activity degree,flexion deformity and stability were significantly changed（P〈0.01）.Pain and muscle strength were little improved（P〉0.05）.The excellent and good rate of treatment group were greater than that of the control group and there were significant differences（P〈0.05）.The total effective rate in the treatment group was 91.67%,for control group,the total efficiency of was 81.67%.There were significant differences of the therapeutic efficacy between the two groups.Conclusion：Integral syndrome differentiation in treating knee osteoarthritis treatment is safe and effective,and easy to be popularized and applyed.
