The health seeking behaviors of AIDS patients with oral disease

目的调查AIDS患者口腔疾病的发生率、就医率、就医去向和接诊状况,探讨口腔门诊预防AIDS的必要性和重点防范场所,以及诊治AIDS口腔病损的能力。方法用自制调查表对湖北省9个市、县的369例患者进行调查,并对结果进行统计分析。结果①AIDS患者发生口腔病损和牙问题的频率高(63.4%、51.1%),就医率高(90.0%、94.0%),就医时间多在上午9～10点。②主要就医场所是个体或乡镇、社区口腔门诊,其次是AIDS定点医院口腔门诊。③创伤性治疗多,认为治疗效果好的人少(22.2%)。④就医时78.2%的患者未出示AIDS确诊结果,99.5%未被告知患有AIDS,被询问AIDS病史、病情及做相关检查的很少(26.7%、24.5%、1.9%)。结论口腔门诊有传播HIV的隐患,其诊治AIDS口腔病损的能力有待于加强,个体或乡镇、社区口腔门诊是高危险诊疗场所,需重点整改。

Objective To investigate the incidence and medical consultation rate of oral disease in patients with AIDS,so as to explore the necessity and main settings of AIDS prevention in dental clinics ,and analyze the capacity to diagnose and treat oral diseases for AIDS patients. Method Totally 369 patients in nine cities and counties in Hubei province were investigated with a self-designed questionnaire. Results AIDS patients had a high incidence of oral lesions（63.4 percent） and dental problems（51.1 percent）,and medical consultation rate was high. Most of them went to the clinic from 9am to 10am. The main settings for medical consultation were private,township and community dental clinics,followed by dental clinics in general hospitals. Many of them received traumatic treatment,but few of them thought the treatment was effective（22.2 percent）. During medical treatment,78.2 percent of the patients with AIDS did not present the results of AIDS diagnosis; 99.5 percent of the pa- tients were not informed about the diagnosis of AIDS,few of them were asked the history of AIDS,the condition of the disease and whether the relevant examination were made or not（26.7 percent,24.5 percent, 1.0 percent,respectively）. Conclusions There are hidden risks of spreading HIV in dental clinics. The capacity to diagnose and treat oral lesions and dental diseases for patients with AIDS needs to be improved. The private,township and community dental clinics are high risk settings.
