房角金环植入术治疗顽固性青光眼的临床观察

Clinical observations of gold ring implantation through anterior chamber angle for the treatment of refractory glaucoma

目的 观察房角金环植入术治疗顽固性青光眼的近期和中期疗效。方法 对29例(29只眼)药物难以控制的青光眼行房角金环植入术。结果 随访2～23月,平均73±53月。眼压由术前(5134±1369)mmHg降至术后(2659±1127)mmHg。总成功率为7241%,其中非新生血管性青光眼的成功率为7692%,新生血管性青光眼的成功率为3333%。并发症包括前房积血、浅前房、术后早期低眼压、前部葡萄膜炎、金环与角膜内皮接触、金环外露、渗出性脉络膜脱离等。结论 房角金环植入术是一种治疗顽固性青光眼的有效方法。

ObjectiveTo investigate the short term and mid term efficacy of gold ring implantation through anterior chamber angle for refractory glaucoma.Methods29 cases (29 eyes)with glaucoma whose intraocular pressure(IOP) had not been controlled satisfactorily with antiglaucoma drugs underwent gold ring implantation through anterior chamber angle.ResultsAfter (7 3±5 3) months of follow up(range 2 to 23 months),the IOP was decreased from preoperative (51 34±13 69)*!mmHg to postoperative (26 59±11 27)*!mmHg.The total success rate was 72 41%,76 92% for non neovascular glaucoma and 33 33% for neovascular glaucoma.The complications included hyphema,flat anterior chamber,early postoperative hypotony,anterior uveitis,implanted gold ring touch to cornea,implant exposure,exudative choroidal detachment and so on.ConclusionGold ring implantation through anterior chamber angle is effective in the management of refractory glaucoma.
