慢性阻塞性肺病运动与恢复期呼吸方式的改变

摘要 目的 :探讨慢性阻塞性肺病 (COPD)极量运动及其恢复期呼吸方式的改变。方法 :临床稳定期COPD患者 40例和正常中老年男性 2 2例作极量运动负荷试验。以运动及其恢复期潮气量 (VT)作为横座标与分时通气量 (·VE)作为纵座标作图 ,比较运动及其恢复期呼吸方式的改变。结果 :与正常人相比 ,COPD患者△VT显著增加 ,其大小与FEV1%和VEmax/MVV明显相关 ,但与 ·VO2 max、·VO2 max/Pred·VO2 max、HRmax、SaO2 min、VTmax、EqO2 max、EqCO2 max等无明显的相关性。结论 :临床稳定期COPD患者极量运动恢复期存在浅快呼吸 ,其发生可能与呼吸肌疲劳等有关。 ?Objective:To investigate changes of breathing patterns in the patients with chronic obstructive pulmonary disease during and after maximal exercise.Methods:40 patients with stable chronic obstructive pulmonary disease and 22 normal middle aged and aged subjects underwent maximal exercise test.Breathing patterns during and after exercise were measuned by plotting the tidal volume(VT) on the abscissa and minute ventilation(AV·UE) on the ordinate.Results:In comparing with the normal controls,△VT in the patients with chronic obstructive pulmonary disease was significantly increased and related to FEV\-1% and AV·UEmax/MVV but not related to AV·UO\-2max,AV·UO\-2max/predicted AV·UO\-2max,HRmax,SaO\-2min,VTmax,EqO\-2max,or EqCO\-2max.Conclusions:Rapid shallow breathing after maximal exercise in chronic obstructive pulmonary disease may be due to fatigue of the inspiratory muscle.
