摘要 目的评价急性心肌梗死(AMI)并泵衰竭的患者早期应用主动脉球囊反搏术(IABP)对心功能的影响。方法选取AMI并泵衰竭、入院后6h内无心源性休克发生的患者共计54例,其中IABP组27例,在直接PCI、积极药物治疗基础上早期应用IABP辅助治疗;对照组27例,给予直接PCI及积极药物治疗,若病情进展至心源性休克,则给予IABP治疗。比较两组患者住院24h内及发病后随访6月时超声心动图及血浆BNP水平。结果 IABP组与对照组24h内纽约心功能分级、左心室功能指标及血浆BNP水平无显著差异(P﹥0.05),但发病6月IABP组BNP水平显著低于对照组(P﹤0.05),超声心动图示左心室功能指标IABP组显著优于对照组(P﹤0.05)。结论早期应用IABP能显著改善AMI并泵衰竭患者心功能。 [ Objective] To investigate the effect of early usage of intra- aortic balloonpump (IABP) on cardiac function in patients with acute myocardial infarction (AMI) complicated by pump failure. [ Methods ] 54 patients with AMI complicated by pump failure underwent primary PCI were randomly divided into two groups: the IABP group ( N = 27 ) received early IABP support and the control group ( N = 27) received IABP support only when aggravated to cardiac shock. The ultrasonic indexes and the serum BNP level were measured after 24 hours and 6 months after ad- mitted. [ Results ] The ultrasonic indexes or the serum BNP level did not show significant differences between the two groups 24 hours after admitted ( P 〉 0.05). However, at sixth month, compared with control group, the IABP group showed significantly improved unltrasonic indexes and the serum BNP level (P 〈 0.05 ). [ Conclusion] Early usage of intra- aortic balloonpump (IABP) improved cardiac function in patients with acute myocardial infarction (AMI) complicated by pump failure.
