摘要
目的研究肺部良恶性肿瘤多层螺旋CT(MSCT)各灌注参数的改变和良恶性肿瘤的鉴别诊断。方法 40例患者中良性病变组6例、恶性肿瘤(肺癌)组34例,均行肺血流的CT灌注扫描。应用去卷积算法模式计算相应病变区域的血流量(BF)、血容量(BV)、平均通过时间(MTT)、毛细血管通透性(PS)。根据不同肿瘤的相关参数图来评价良恶性肿瘤血流动力学状态。结果 34例肺癌患者BF、BV和PS明显高于良性病变,其中PS值>25[mL·(100g·min)-1]的敏感性为97.06%、特异性为83.33%,而MTT值与良性病变无显著差异。结论灌注参数BF、BV和PS可有效的评价肺部病变的血流状态,在肺部良恶性病变的鉴别诊断上有重要的临床应用价值。
[ Objective ] To study MSCT perfusion imaging of fine or malignancy tumor of lung and differentiate di- agnose. [ Methods ] dynamic CT with singe - location sequences CT scan of the lung were obtained in 40 individuals who include 6 patients with fine process and 34 patients with malignancy tumor (lung cancer). Lung perfusion parame- ters include blood flow (BF), blood volume (BV), mean transit time (MTT) and permeability surface (IX3). [ Re- sults ] Compared with fine process of lung, BF, BV and PS in lung cancer group increased significantly, The PS 〉 25 [ mL.( 100g. min) -1 ] was set as a diagnostic threshold, the sensitivity and specificity were 97.06% and 83.33% , and MTT has not differ between two group. [ Conclusion] Lung perfusion parameters (BF, BV and PS) measured with CT were significantly altered in lung tumor, There have importance appliance value in differentiate diagnose of fine or malignancy tumor.
