脂联素与冠脉临界病变斑块易损性的相关性分析 被引量:4

摘要 目的:通过将冠心病患者血浆脂联素(APN)水平与冠脉血管内超声结果进行相关性分析,初步探讨APN对冠脉临界病变斑块易损性的预测价值。方法:采用放射免疫法测定经冠脉造影证实的16例冠脉正常者(对照组)与32例冠心病患者(冠脉直径狭窄40%~70%)的空腹血浆脂联素水平,同时检测血脂、血糖、体重指数(BMI)等指标的水平。32例冠心病患者采用血管内超声测定冠脉斑块的易损性,评价脂联素与冠脉斑块易损性的相关性。结果:(1)稳定斑块组甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)较易损斑块组低(P<0.05),高密度脂蛋白胆固醇(HDL-C)较易损斑块组高(P<0.05)。(2)APN与LDL-C、TC、TG呈负相关(r=-0.633,r=-0.359,r=-0.304,P值均<0.05),与HDL-C呈正相关(r=0.784,P<0.05)。(3)APN水平与斑块易损性呈显著负相关(r=-0.559,P<0.01),其敏感性、特异性、阳性预测值、阴性预测值分别为85.7%、66.7%、76.79%、78.39%。结论:检测血浆脂联素水平对冠脉斑块易损性具有一定的预测价值。
