胃粘膜相关淋巴组织淋巴瘤的内镜下表现

Gastric mucosa associated lymphoid tissue lymphoma :endoscopic features

目的总结胃粘膜相关淋巴组织淋巴瘤(MALT淋巴瘤)的内镜特征。方法对19例胃MALT淋巴瘤患者的临床资料进行回顾分析。结果胃MALT淋巴瘤的内镜下表现呈多样性改变 ,病变累及胃体最多(78.9 %) ,主要病变形态包括溃疡(63.2 %)、肿块(15.8 %)、浸润病变(15.8 %)及糜烂(5.3 %)。大多数患者 (73.7 % )有较典型的恶性征象 ,但少数可无典型恶性征象 ,甚至仅表现为一般的炎症及糜烂。本组病例内镜活检确诊率高达84.2 % ,内镜下多次多点活检 ,深凿活检及粘膜切除法(EMR)活检可提高本病的早期诊断率。

Objective The endoscopic aspects of gastric mucosa associated lymphoid tissue ( MALT ) lymphoma was studied . Methods The clinical and endoscopic materials of 19 patients with gastric MALT lymphoma was retrospectively reviewed and analysed. Results MALT lymphoma invloved most commonly the body of stomach ( 78.9%) . Its endoscopic appearance was multimorphic including ulcerative ( 63.2%) , protruding ( 15.8%), infiltrative ( 15.8%) and erosive ( 5.3%) types. most presented signs of typical malignant growth with few untypical outlook like erosions . Positive findings by biopsies attained 84.2%, the percentage rose up with more biopsies taken from multiple sites , mucosal strips and deep bites Conclusion Gastroscopy remains to be valuable to set correct diagnosis of gastric MALT lymphoma .
