阻塞性睡眠呼吸暂停低通气综合征患者视黄醇结合蛋白-4水平及其与胰岛素抵抗的关系

摘要 目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者血清视黄醇结合蛋白-4(retinol bind-ing protein 4,RBP4)水平和胰岛素抵抗程度的相互关系。方法 OSAHS患者41例晨起后收集血清测量胆固醇、三酰甘油、RBP4、脂联素,测定空腹血糖浓度、胰岛素水平(FINS)。采用稳态模型评估法(HOMA)评价并计算胰岛素抵抗指数(HOMA-IR),并与23例非OSAHS患者进行比较。结果 OSAHS组血清RBP4水平、夜间最低血氧饱和度(MinSaO2)、脂联素均低于对照组。OSAHS组的血氧饱和度低于90%的时间占总睡眠时间百分比(TSaO2<90%)、AHI、BMI、TG、FINS、HOMA-IR皆显著高于对照组,差异有统计学意义。RBP4水平与HOMA-IR、AHI、TSaO2<90%、MinSaO2不存在相关性。结论脂联素是影响OSAHS患者血清RBP4水平的主要因素,单纯的血清RBP4可能不是评估OSAHS患者胰岛素抵抗的有效指标。
