不同导向经皮肺穿刺活检并冲洗对周围型肺肿块的诊断价值

Detection of Patients with Peripheral Pulmonary Swollen Mass by Lung Biopsy and Flushing in Different Directions

对32例常规检查不能确诊的周围型肺肿块,采用不同导向经皮肺活检。结果25例由病理确诊,7例经冲洗液涂片及测定肿瘤标记物而确诊;肺癌16例,肺结核13例,肺炎3例。肺癌组冲洗液中癌胚抗原(CEA)、糖链抗原153(CA153)、非小细胞肺癌抗原(LTA)水平明显高于炎症组(分别为P<001,P<005,P<001)。表明B超导向经皮肺活检安全、方便、准确,冲洗液中多项指标联检能提高组织细胞学诊断率,弥补纤支镜检查的不足。

patients with peripheral pulmonary swollen mass that can't be ieagnosed in conventional ways were studied by lung biopsy and flushing in different directions Rusult:25 patients were pathologically diagnosed 7 patients were diagnosed by smear of flushing fluid and assay of tumor marker 16 patients with lung cancer 13 patients with pulmonary tuberculosis 3 patients with pneumonia CEA,CA153 and LTA(P<0 01,P<0 05 and P<0 01 respectively)of lung cancer group in flushing fluid were much higher than those in imflammatory group It indicates that percutaneous pneumocentesis by B ultra sound was safe,convenient and accurate Associated checkout of multiple,indexes in flushing fluid can not only improve diagonstic rate of histiocytology,but also compensate deficiencies of fiber bronchoscope
