摘要 目的探讨CT三维重建在尺骨冠状突骨折治疗中的指导意义。方法对19例尺骨冠状突骨折患者摄X线片、CT扫描及CT三维重建检查,对骨折进行分型,并据此制定最佳治疗方案。结果根据尺骨冠状突骨折的O'Drlscoll分型,CT三维重建图像显示19例尺骨冠状突骨折患者中Ⅰ型骨折6例,Ⅱ型骨折11例,Ⅲ型骨折2例。除2例Ⅰ型冠状突骨折外,余17例均采用了内固定治疗。19例均获得随访,时间6~30个月。骨折均愈合,愈合时间8~12周。疗效根据Morrey et al的肘关节功能诊断标准评定:优12例,良4例,差3例。结论 CT三维重建能多方位、立体、全面地显示冠状突骨折情况,对骨折分型、治疗方案选择有很强的指导意义。 Objective To investigate the clinical effect of three-dimensional CT image reconstruction in the management of unlar coronoid process fracture.Methods 19 cases of unlar coronoid process fracture were examined by X-ray films and three-dimensional CT reconstruction.Based on the images,all fractures were classified and evaluated in order to select the optimal treatment.Results According to O′Drlscoll classification,there were 6 cases of type Ⅰ,11 of type Ⅱ,and 2 of type Ⅲ,17 cases were performed operations.Nineteen cases were followed up for 6~30 months.All fractures healed well,and the average union time was 10 weeks with a range of 8~12 weeks.The clinical results were evaluated according to Morrey′s scale,and 12 cases were rated as excellent,4 good,and 3 fair.Conclusions Three-dimensional CT reconstruction images provide excellent fracture detail,which is important for fracture classification and preoperative planning.
