导致异位妊娠术中低血压因素的回顾性分析

A respective analysis on the factors causing hypotensions in ectopic pregnancy operations

目的寻找和分析异位妊娠手术中造成低血压的因素。方法回顾性分析某院2005—2006年问242例异位妊娠手术患者的资料，对可能造成血压波动的因素进行筛选。入选变量有体重、年龄、麻醉方法、术前休克指数、围术期输液量、术前MAP和围术期输血量等7个变量，经Logistic多元回归技术进行逐步回归，建立回归方程。结果经回归分析，术前休克指数、术前平均动脉压和不同麻醉方法等三个因素有统计学意义（P〈0．05）。结论术前休克指数增大、术前平均动脉压偏低和使用硬膜外麻醉使异位妊娠手术中发生低血压的几率增加。

Objective To observe and analyze the factors that may lead to the hypotension in ectopic pregnancy operations. Methods 242 patients in The First Affiliated Hospital of Xinjiang Medical University from 2005 to 2006 were retrospectively studied. Seven affecting factors were found to he age, weight, anesthesia method, preoperative shock index, preoperative transfusion volume, preoperative mean arterial pressure （MAP） and blood transfusion volume. A logistic model was developed to evaluate predictors of intraoperative hypotension. Results Preoperative shock index, preoperative MAP and different anesthesia methods had significant difference in ectopic pregnancy operations（P 〈 0.05）. Conclusions Higher preoperative shock index, lower preoperative MAP and epidural anesthesia can cause the increasing incidence rate of hypotension in ectopic pregnancy operations.
