老年单纯收缩期高血压颈动脉重构与脉压及基质金属蛋白酶-9的相关性研究

Relationship between remodeling of the carotid artery and pulse pressure,MMP-9 in the elderly patients with ingenuous systolic hypertension

目的探讨老年单纯收缩期高血压(ISH)患者颈动脉内膜-中层厚度(IMT)与脉压(PP)及血清基质金属蛋白酶-9(MMP-9)之间的相关性。方法收集ISH患者52例(ISH组)、混合性高血压(SDH)患者48例(SDH组)以及老年健康体检组50例(健康对照组),分别检测脉压(PP)、血MMP-9及IMT,分析三者之间的关系。结果 ISH组、SDH组PP、MMP-9水平及IMT均高于健康对照组(P<0.01),且ISH组较SDH组高(P<0.05),IMT与SBP、DBP、PP及MMP-9呈正相关(r=0.333,r=0.263,r=0.573,r=0.742,P<0.01)。多元逐步回归分析表明,PP、MMP-9为IMT的独立危险凶素(P<0.05)。结论 ISH组患者颈动脉重构程度较SDH组重,PP及MMP-9水平升高与IMT增厚呈正相关,PP及MMP-9升高可能是造成动脉粥样硬化的重要因素。

Objective To investigate the relationship between the intima media thickness （IMT） of the carotid artery and pulse pressure（PP）, matrix metalloproteinase-9 （MMP-9） in the elderly patients with ingenuous systolic hypertension （ISH） . Methods The levels of PP, MMP-9 and the IMT of carotid artery were detected and analyzed statistically in 52 elder ISH patients, 48 systolic and diastolic hypertension （SDH） patients and 50 healthy controls. Results The levels of PP andMMP-9 in two hypertension groups were significantly higher than those in normal controls （ P 〈 0.01 ）, while those was higher in ISH group than SDH group （ P 〈 0.05 ）. the IMT of carotid artery was higher than that in normal controls, the levels of PP and matrix metalloproteinase-9 （MMP-9） in the ISH group were higher than those in SDH group（ P 〈 0.05 ）. Positive rela- tionships were found between IMT and SBP, DBP, PP, and MMP-9 （ r -- O. 333, r -- O. 263, r = O. 573, r --- O. 742, P 〈 0.0l）. Multiple regression analysis showed that PP,MMP-9 were independent risk factors of IMT（ P 〈0.05）. Conclusion In ISH patients, IMT level＇is correlated with level of MMP-9 and PP,PP and MMP-9 may be the risk factors of vascular remodeling.
