GammaⅢ髓内钉微创治疗老年股骨转子间骨折

Clinical analysis of minimally invasive plate osteosynthesis with GammaⅢ nail screw in treatment of intertrochanteric femoral fracture in the old patients

目的探讨GammaⅢ髓内钉微创治疗老年股骨转子间骨折的临床疗效。方法 35例老年股骨转子间骨折患者采用C臂机透视下牵引复位,微创切口GammaⅢ髓内钉治疗。结果 35例患者均骨性愈合,愈合时间3个月~8个月,随访6个月~24个月,未出现退钉及断钉病例,未出现畸形愈合病例。疗效评定优24例、良8例、可3例,优良率为92%.结论GammaⅢ钉微创治疗老年股骨转子间骨折手术时间短,出血少,创伤小,固定牢固,是老年股骨转子间骨折的理想治疗方法。

Objective This retrospective study is to evaluate the therapeutic efficacy of GammaⅢ nail applied for intertrochanteric femoral fracture of the old by minimally invasive plate osteosynthesis.Methods 35 case of patients with intertrochanteric hip fractures were treated by minimally invasive plate osteosynthesis with GammaⅢ nail from february 2008 to august 2010.Results The operation time was 60 min on average（40 min^90 min）.Blood loss was 120 mL on average（100~300 mL）.Intraoperative deaths were not discovered.There were 2 cases of postoperative pulmonary infection and 1 cases of urinarytract infection.All patients were followed up for 6 to 24 months.There were 25 cases of excellent effect.9 cases of good effect and 3 Cases of fair effect The good rate was 92%.All patients were healed within 3 to 8 months.Neither breakage of internal fixation nor varns of hip was discovered.Conclusion GammaⅢ nail is an ideal internal fixator for intertrochanteric hip fractures of the old patients for short operation time-less bleeding,small trauma and stable fixation.
