中颅窝底及侧裂内血肿的手术治疗

The surgical therapy of middle fossa and sylvian sulcus hematoma

目的 :探讨中颅窝底及侧裂内血肿的手术要点 ,以期提高该病种预后质量。方法 :对 6 7例中颅窝底及侧裂内血肿的术前、术后临床表现以及术中特征结合解剖学进行回顾性分析。结果 :42例获得了相对满意的生存质量 ,19例遗留有不同程度的神经缺损体征 ,死亡 6例。结论 :对 2 0 ml以上的中颅窝底或侧裂内血肿应积极手术治疗。在保护重要神经功能的基础上 ,充分暴露颅底或侧裂 ,确切止血是手术成功的关键 ,术后应适当延长引流时间。

Objective To evaluate surgical therapy of middle fossa and slyvian sulcus hematoma.Methods With the local anatomic knowledge,We retrospective analyzed the clinic data of 67 patients with middle fossa and sylvian sulcus hematoma.Results Of the 67 patients,42 patients improved their life quality,19 patients left neurological deficiency in a certain degree,6 patients died.Conclusions It is important to proceeded operation earlier for the patients with middle fossa and sylvian sulcus hematoma more than 20 ml.The good exploration and accurate hemastatin are the key points of the operation.
