产时会阴Ⅱ度复杂裂伤双合诊修复缝合的临床观察

目的:探讨产时会阴Ⅱ度复杂裂伤的修复缝合方法及预后情况,以利于今后对于此类裂伤修复进行指导。方法:收治阴道分娩发生产时会阴Ⅱ度复杂裂伤患者125例,对资料进行回顾性分析,对比下列项目:缝合时间、术后病率、术后生活质量等进行对比观察。结果:双合诊单层缝合阴道法较直视缝合法显著缩短缝合时间,降低了术后病率,不影响术后生活质量。结论:产伤的原因包括母体因素(会阴发育不良及有关病变,阴道炎、外阴炎);胎儿因素(畸胎、巨大儿、胎位异常);医源性因素(产科手术、急产等)。医护人员应尽心尽责,提高医护质量,正确的判断和掌握手术指征,认真仔细地进行技术操作,产伤均可减少或避免。

Objective：To explore the repairing and suture methods and prognostic effects on the delivery of the perineal complicated laceration with degree II for guiding the treatment of this kind in future.Methods：The retrospective analysis was performed on 125 cases with the vaginal delivery of the perineal complicated laceration with degree Ⅱ from January 2008 to December 2011 in our hospital to observe their suturing time,their follow-up morbidity and their life quality.Result：Compared with the suture under the direct vision,the combination diagnosis on single-layer anastomosis of vagina can greatly shorten the suturing time,decrease the follow-up morbidity,and have no influence on the life quanlity.Conclusion：The causes of laceration include the maternal factors（perineal dysplasia and lesions,vaginitis,vulvitis）;fetal factors（deformity,fetal macrosomia,abnormal fetal position）;iatrogenic factors（obstetric surgery,precipitate labor and so on）.In order to decrease and avoid laceration,deep concern of the medical staff is necessary.It is also required that the medical quality should be improved and the surgery should be under the correct judgmentand control of the surgical indication.
