摘要
目的:研究肺气肿患者肺叶水平的血流灌注变化与肺气肿严重程度的关系,探讨多排螺旋CT肺灌注成像在评价肺气肿严重程度中的应用价值。方法:对14例正常者和10例肺气肿患者行64排螺旋CT肺灌注成像,针对其组织强化峰值(PE)进行定量分析,考察其肺血流动力学变化情况。结果:正常组肺血流灌注呈现从上向下逐渐增多的分布趋势。与正常组比较,肺气肿组肺灌注强化峰值PE下降,差异具有显著性(P<0.05);且肺实质破坏与肺灌注减低的分布区域和程度相匹配。未发现肺气肿者肺功能GOLD分级与全肺灌注总强化值之间存在相关性(P>0.05)。结论:多排螺旋CT肺灌注成像可用于临床对肺气肿的定量分析和评价。
Purpose:To study the relationship between alterations in lung perfusion and parenchymal destruction of emphysema patients by using multi-slice CT perfusion imaging,and to explore its clinical value on evaluation of emphysema.Methods:Fourteen normal controls and ten emphysema patients were enrolled in this study.64-slice CT perfusion imaging was used to measure the peak enhancement(PE).The pulmonary hemodynamic changes were evaluated.Results:The regional pulmonary perfusion in the upper lobes was significantly lower than that of the middle and lower lobes in 14 normal controls.Significantly lower peak enhancement values were found in emphysema patients when compared with normal controls(P〈0.05),which were matched the parenchymal destruction.We found no correlation between the disorder of lobular perfusion and the GOLD stages in emphysema patients(P〉0.05).Conclusion:Multi-slice CT perfusion imaging has the potential to detect pulmonary hemodynamic defects in patients with predominant emphysema.
