临床药师介入用药医嘱点评及其分析

Review and Analysis of Clinical Pharmacist's Influence on Doctor's Orders for Drug Use

目的了解住院患者用药医嘱存在的问题,探索临床药师介入用药医嘱点评工作的模式。方法依据《处方管理办法》,对2010年3月～2011年2月点评的378份病例资料进行回顾性分析。结果不合理用药医嘱254条,占用药医嘱总条目数(18 763)的1.35%。主要包括药物用法用量不恰当、溶媒种类及剂量不当、配伍禁忌、重复给药等问题。结论通过临床药师对用药医嘱的审核,可以及时发现不合理用药现象,促进临床合理用药。

Objective To probe into the existing problems in drug use of and the operating mode of clinical pharmacist influence in Review and Analysis on Drug Use of Doctor＇s Orders.Methods The analysis of 378 cases from March 2010 to Feb 2011 were sampled for statistical analysis according to the Prescription Management Method.Results There are 254 unreasonable doctor＇s orders with failure rate of 1.35%（the total number is 18 763）.The unreasonable doctor＇s orders were mainly manifested as the incorrectness of dosage and administration,incorrect type and dosage of medium,incompatibility,and repeated medication.Conclusion Reviewing drug use of doctor＇s orders by clinical pharmacist could find unreasonable drug use in time,and it could improve drug use in clinical practice.
