ω-3多不饱和脂肪酸对重型颅脑损伤患者营养状况及免疫学指标的影响 被引量:4

摘要 目的探讨ω-3多不饱和脂肪酸(ω-3 PUFA)对重度颅脑损伤患者营养状况及免疫功能的影响。方法将34例重型颅脑损伤患者随机分为常规胃肠外营养组(TPN组)及ω-3 PUFA+TPN治疗组(PUFA组),每组17例。两组患者于治疗前与治疗后第3和7天检测血前白蛋白(PAB)、白蛋白(ALB)、转铁蛋白(TF)及血糖(GLU),同时检测IgA、IgG、IgM的变化,并比较两组病人感染发生率及ICU入住时间。结果与TPN组比较,PUFA组血清IgA、IgG、IgM明显升高(P<0.01或0.05);PAB、ALB及TF水平明显升高(P<0.01或0.05),而GLU水平明显下降(P<0.01或0.05);ICU入住时间明显缩短(P<0.05),但感染发生率两组比较,差异无统计学意义(P>0.05)。结论ω-3 PUFA可显著改善重型颅脑损伤患者的营养状况,提高免疫功能,从而改善患者的预后。 Objective To investigate the effects of ω-3 polyunsaturated fatty acid(ω-3 PUFA) on nutritional and immune status in patients with sever brain injury.Methods 34 patients with sever brain injury were assigned randomly into 2 groups(n=17 in each): conventional total parenteral nutrition group(TPN group) and ω-3 PUFA+TPN group(PUFA group).The nutritional indicators include serum pre-albumin(PA),albumin(Alb),transferring(TF) and glucose(GLU),and immunological indicators including IgA,IgG and IgM were measured before and 3rd,7th day after parenteral nutrition.The infection rate and the time in ICU were also recorded.Results Compared with group TPN,the levels of IgA,IgG,IgM,PA,Alb and TF in PUFA group were increased significantly(P0.05) TPN.Conclusion The application of ω-3 PUFA may improve nutritional status and immune status of patients with sever brain injury,thus improving the prognosis of patients.
