杭州地区军队老年人群糖尿病流行病学调查及评价

Epidemiological investigation and evaluation of military elderly population with diabetes in Hangzhou

目的调查分析杭州地区军队老年人群糖尿病的患病状况,分析与伴随疾病谱的关系,评价药物疗效。方法采用病史回顾与体检相结合方法将726例老年人群分为糖尿病组和非糖尿病组,分析两组血压、血脂及伴随疾病谱以及糖尿病组使用药物治疗状况。结果杭州地区军队老年人群中糖尿病患病率为40.63%,与非糖尿病组相比,两组人群血压、总胆固醇水平无统计学差异,但三酰甘油水平有显著差异(P<0.05);糖尿病组并发高血压、冠心病、慢性肾功能不全、脑卒中及恶性肿瘤的比例明显高于非糖尿病组(P<0.05或P<0.01);糖尿病病程≥10年以上的使用胰岛素比例为31.71%(72/227),明显高于病程<10年的11.76%(8/68),有明显差异(P<0.05)。结论老年人群中2型糖尿病血管病变导致并发高血压、冠心病、慢性肾功能不全、脑卒中及恶性肿瘤的比例明显增高,且应用胰岛素治疗的比例随病程的延长而上升。

Objective To investigate the prevalence of the military elderly population with diabetes in Hangzhou,analyze their relationship with disease spectrum and evaluate the drug efficacy.Methods According to the medical history review and physical examination,726 cases of military elderly population were divided into diabetic and non-diabetic group.The blood pressure,lipids,associated disease spectrum of the two groups and the drug efficacy for diabetes group were analyzed.Results The prevalence rate of diabetes was 40.63% in the military elderly population in Hangzhou.Compared with non-diabetic group,there was no significant statistical difference between the two groups such as blood pressure,total cholesterol levels,but the difference of triglyceride levels was significantly（P 〈0.05）.The rate of diabetic with hypertension,coronary heart disease,chronic renal failure,stroke,and malignant tumor were significantly higher than non-diabetic group（P〈0.05）;the rate of diabetes who were using insulin and their duration≥10 years,was 31.71%（72/227）,significantly higher than 11.76%（8/68） of diabetes whose duration 〈 10 years,there were statistical significant differences（P〈0.05）.Conclusion Diabetic vascular disease leads to high prevalences of hypertension,coronary heart disease,chronic renal failure,stroke and malignant tumors.And the proportion using insulin therapy increased with the extended duration.
