支架辅助弹簧圈栓塞治疗颅内宽颈动脉瘤的应用研究

Application of stent-assisted coil embolization therapy on intracranial wide-necked aneurysm

目的评价支架辅助弹簧圈栓塞治疗颅内宽颈动脉瘤的有效性和安全性。方法回顾2009年9月至2011年9月间应用支架辅助弹簧圈栓塞治疗颅内宽颈动脉瘤18例患者的临床资料,分析动脉瘤与载瘤动脉及载瘤动脉解剖形态,随访观察治疗效果,总结操作经验。结果所有动脉瘤均成功栓塞,术后即刻栓塞16个,瘤颈残留1个,部分栓塞1个,术中弹簧拉丝1例,没有血栓栓塞并发症,平均10个月后数字成影血管造影(DSA)随访。结论支架辅助弹簧圈栓塞是治疗宽颈动脉瘤的重要手段,可提高宽颈动脉瘤颈覆盖率,值得在临床上推广应用。其治疗效果需长期随访和评价。

Objective To observe the efficacy and safety of stent-assisted coil embolization therapy on inracrani-al wide-necked aneurysm.Methods A retrospective review of 18 patients with wide-necked intracranial aneurysms treated by stent-assisted coil embolization therapy from 2009 to 2011 was conducted to analyze the procedures and morphology of aneurysm and the artery.The efficacy and operation experience were analyzed. Results Embolization succeeded technically in all of the aneurysms.Embolization after operation was conducted immediately in 16 aneurysms,with residual neck in 1,part-embolization in 1 and one case with intraoperative spring wire drawing.There was no thrombosis.Angiographic follow-up were carried out in cases for 10 months. Conclusion The results suggested that stenting and stent-assisted coil embolization is an important therapy on intracranial wide-necked aneurysm.It can improve neck cover for vascular reconstruction.It is worth clinical application and needs long term follow-up and evaluation.
