长脉冲Nd:YAG激光“点阵式”治疗面部血管瘤疗效观察

Treatment of Facial Hemangioma by Using Long Pulse Nd:YAG Laser in Fractional Way

目的:探讨使用长脉冲Nd:YAG激光治疗面部血管瘤的新方法。方法:68名患儿,共68个皮损,血管瘤直径1.0～3.2cm,平均2.2cm,随机分为传统组和点阵组,将脉冲Nd:YAG激光(参数:能量密度160～220mJ/cm2,脉冲宽度15ms,光斑直径3mm)光斑分别以传统整片覆盖方式及点阵式作用于血管瘤表皮,根据表皮变化调整能量,治疗终点为每脉冲激光作用后表皮变灰白。术后予局部防水、外涂金霉素软膏处理。治疗间隔1～2个月,治疗次数3～5次。结果:两种治疗方式治疗后血管瘤都有不同程度的改善,传统组有效率为96.77%,点阵组有效率为97.30%,点阵式相比于传统方式疗效更显著(P<0.05),且点阵组出现并发症的几率较传统组少。结论:长脉冲Nd:YAG激光以点阵式治疗面部血管瘤较传统方式效果好,值得临床推广。

Objective To explore a new method to treat facial hemangioma by using Nd：YAG laser. Methods 68 patients with 68 treatment areas were included in this study and divided randomly into two group, traditional group and fractional group. The treatment areas were radiated by pulse Nd：YAG laser （Treatment parameters： Fluence 160-220mJ/ cm2,Pulse Width 15msec, Spot Diameter 3mm） in fractional and traditional way respectively. Waterproof care and aureomycin ointment was used postoperation. The treatment intervals were 1 -2months and there would be 3-5 processes. Results Two treatment groups got different clinical efficacy, the effective rate of traditional group is 96.77%, and the fractional group is 97.30%,areas treated by fractional way got more remarkable effect （P〈0.05）with less complications. Conclusion Using Nd：YAG laser in fractional way is a good way to treat facial hemangioma with less complications.
