氯丙嗪与硝苯地平对妊娠期高血压疾病产后降压治疗效果的比较

Comparative study on postpartum antihypertensive therapy in patients with hypertensive disorder complicating pregnancy by Chlorpromazine and Nifedipine

目的探讨盐酸氯丙嗪与硝苯地平治疗对妊娠期高血压疾病病例产后降压治疗的临床疗效。方法 96例有降压治疗指征的产后妊娠期高血压疾病病例,随机分为观察组和对照组各48例。观察组予硫酸镁15g解痉治疗1次,同时给予盐酸氯丙嗪片25mg,口服,1次/d,共3d。对照组予硫酸镁15g解痉治疗1次同时给予硝苯地平控释片30mg/d,口服,1次/d,共3d。结果观察组48例中,治疗总有效率为91.7%(显效32例,有效12例);对照组48例中,治疗总有效率为70.8%(显效19例,有效15例)。观察组收缩压、舒张压控制效果优于对照组,差异具有统计学意义(P<0.05)。观察组病例产后24小时平均出血量(350±154)ml,平均心率(92±24)bpm。对照组病例产后24小时平均出血量(387±175)ml,平均心率(98±26)bpm。两组相比较差异无统计学意义(P>0.05)。结论盐酸氯丙嗪对妊娠期高血压疾病病例产后降压治疗效果优于硝苯地平,能有效降低产后血压,具有较好的临床应用价值。

Objective To study the efficacy of postpartum antihypertensive therapy with Chlorpromazine and Nifedipine in patients with hypertensive disorders in pregnancy（HDP）.Methods 98 cases of postpartum PDH which had indication of antihypertensive therapy were randomly divided into study group（n=48） and control group（n=48）.Cases in study group received magnesium sulfate 15g once time intravenously and chlorpromazine hydrochloride tablet 25mg orally one time daily for three days.Case in control group received magnesium sulfate 15g once time intravenously and Nifedipine controlled release tablets 30 mg orally one time daily for three days.Results The total effective rate of treatment in study group was 91.7%（32cases were marked effect and 12 cases were effect）.The total effective rate of treatment in control group was 70.8%（19 cases were marked effect and 15 cases were effect）.The blood pressure of patients in study group was efficiently controlled than that of control group,and their difference was statistically significant（P0.05）.The amount of pastpartum hemorrhage was 359.1±136.6 ml in study group which had no statistical significance than that of control group（387±175ml）（P0.05）.The mean heart rate was 92± 24 bpm in study group which had no statistical significance than that of control group（98±26bpm）（P0.05）.Conclusions The efficacy of chlorpromazine hydrochloride tablet is better than that of Nifedipine in treatment of hypertensive disorders in pregnancy.It has a broad clinical application.
