我院门诊西药房退药情况分析

目的分析门诊西药房退药情况,了解退药原因及特点,总结经验,以采取相应措施减少退药事件的发生,促进临床合理用药。方法采用回顾性研究方法,收集2011年度门诊西药房退药处方658张,按退药科室、退药原因及退药种类进行统计分析。结果引起门诊患者退药主要有五个方面的原因,其中,药物不良反应217例,占32.98%;药师审方后退回不合格处方187例,占28.42%;医师处方剂量错误147例,占22.34%;患者自身原因63例,占9.57%;其他原因44例,占6.69%。结论患者用药后出现不良反应是退药的主要原因,医师在为患者开具处方时,应详细了解患者的病情,确保合理用药,尽量减少退药事件的发生。

Objective To investigate the reasons of drug repercussion in dispensary for outpatients,to gather experience and to provide references for the reduction and standardization of drug repercussion.Methods Used the retrospective study methods,the causes of drug repercussion in outpatient dispensary in 2011 were investigated and analyzed statistically.Results There were five main reasons caused drug repercussion：ADR was the main reason（32.98%）,returned unqualified prescription after pharmacist checked（28.42%）,error doses of physician prescription（22.34%）,the patient own reasons（9.57%）and the others（6.69%）.Conclusion ADR was the main reason that caused drug repercussion,it is suggested that physicians should investigate patients＇ history of the use and the allergy of drugs in detail.Establish medical plans in accordance with the economy of patients,reduce and standardize drug repercussion to improve the quality of medical service.
