术中造影AF系统内固定术治疗胸腰椎骨折伴不全瘫

Treatment of Thoracolumbar Fracture with Partial Paralysis by Use of Atlas Fixator and Spinal Canal Myelography in Operation

目的 本文报告 2 3例胸腰椎骨折伴不全瘫用AF系统内固定加术中椎管造影的疗效。方法 对 2 3胸腰椎骨折并不全瘫应用AF系统内固定结合术中造影治疗。结果 其中 2 0例占 86 9%造影显示椎管通畅 ,脊髓神经根无受压 ,未行椎板切开减压。 1例硬膜囊破裂 ,1例椎间盘破裂突入椎管内 ,1例椎体游离骨折块突入椎管内。给予有限的椎板减压 ,修补破裂的硬膜囊 ,取出破裂的椎间盘及游离的骨折块。结论 认为该手术创伤小 ,出血少 ,术后恢复好。

Objective This article reported the effect of 23 cases of the thoracolumbar fracture with partial paralysis treated with atlas fixator and spinal canal myelography in operation.Methods Twenty of them,who were of no obstruction of spinal canal and no compression against spinal nerve root,needed no laminectomy. The rest of them,who were of break epidural cavity and of intervertebral disc or isolated fragment of fractured vertebral body bursting into vertebral canal,were given the treatments of limited laminectomy to repair the broken epidural cavity and removal of the broken intervertebral disc and isolated fracture fragment.Conclusion The operation was characteristic of little trauma,less bleeding and good recovery.
