摘要
目的：探讨子宫动脉栓塞术对难治性妇产科大出血疗效的影响.方法：选择2009 年6 月至2011 年6 月难治性大出血患者60 例,随机分为观察组和对照组各30 例,观察组给予动脉栓塞术治疗,对照组给予髂内（或子宫）动脉结扎术治疗,观察两组临床效果.结果：观察组治疗难治性妇产科大出血平均手术时间为42.93±7.1min、平均出血量966.9±227.5ml、术后阴道流血天数11.4±2.6d、子宫切除率6.7%、止血有效率94.8%;对照组治疗难治性妇产科大出血平均手术时间为71.26±10.7min、平均出血量1451.5±250.5ml、术后阴道流血天数16.3±2.6d、子宫切除率33.3%、止血有效率65.7%.两组比较差异有统计学意义（p〈0.05）.结论：子宫动脉栓塞术对难治性妇产科大出血疗效满意,具有手术时间短、止血快、出血量少、并发症低、可保留子宫等优点.
Objective： To explore uterine artery embolization for intractable gynecological massive hemorrhage curative effect. Methods： in 2009June to2011June in refractory patients with hemorrhage in60 cases, were randomly divided into the observation group and the control group with 30 cases in each group, were observed in the groups given arterial embolization therapy, the control group was given the internal iliac （ or uterine artery hgation treatment ）, two groups of clinical effect observation. Results： the observation group was treated for refractory obstetric and Gynecology hemorrhage average operation time was 42.93 ＋ 7.1min966.9 ＋ 227.5ml, the average vol- ume of bleeding, postoperative vaginal bleeding daysl 1.4 ＋ 2.6d, the rate of hysterectomy in 6.7%, hemostatic rate was 94.8%; the control group treatment for refractory obstetric and Gynecology hemorrhage average operation time was 71.26 ＋ 10.Tmin, average bleeding vol- ume1451.5 ＋ 250.5ml, postoperative vaginal bleeding daysl6.3 ＋ 2.6d, the rate of hysterectomy in 33.3%, hemostatic rate65.6%. There was significant difference between two groups （ P 〈 0.05）. Conclusion： uterine artery embolization for intractable gynecological massive hemorrhage the curative effect is satisfied, can shorten the operation time, bleeding fast, less bleeding, less complications, can preserve the uterus and other advantages.
