摘要
目的:主要探究肥胖对Ⅰ型糖尿病影响,为Ⅰ型糖尿病的早期干预提供依据.方法:选择我院2009 年到2011 年间的入院患者共102 例,其中4 例为经典Ⅰ型糖尿病患者,9 例为成人隐匿性自身免疫性糖尿病(LADA)患者,89 例为Ⅱ型糖尿病患者.对其测量身高、体重,计算体质量指数并记录,然后测量糖化血红蛋白、空腹C 肽、空腹血糖、餐后2h 血糖、餐后2hC 肽、谷氨酸脱羧酶抗体、胰岛素、血脂.结果:经与非肥胖Ⅰ型糖尿病患者比较,肥胖Ⅰ型糖尿病易发生冠心病、高脂血症、合并代谢综合征,P〈0.05;空腹、餐后2h C 肽时水平高,P〈0.05;不易出现胰岛素依赖、糖尿病周围神经病变合并,P〈0.05;成人隐匿性自身免疫性糖尿病与肥胖经典Ⅰ型糖尿病比较,后者发病年龄较小,初诊空腹血糖水平较高,易发生酮症酸中毒、胰岛素依赖,P〈0.05;成人隐匿性自身免疫性糖尿病患者与非肥胖经典Ⅰ型糖尿病患者临床特点比较,后者发病年龄小、病程短、初诊时空腹血糖水平高,易出现酮症酸中毒及依赖胰岛素治疗情况,但糖尿病周围神经合并病变、视网膜病变发生几率小,P〈0.05.结论:肥胖性成人隐匿性自身免疫性糖尿病的临床特点同Ⅱ型糖尿病类似[1].
