住院神经症病人噩梦的内容分析

The Nightmare Contents of Hospitalized Neurosis Patients

目的探讨住院神经症病人噩梦的内容特点。方法收集安徽医科大学临床心理学院心理科符合CCM D-3神经症诊断标准的住院神经症病人的噩梦,并以H all and V an de C astle的内容分析方法对噩梦进行编码,将噩梦编码录入D ream sSAT进行自动统计分析。结果共收集40例住院神经症病人噩梦82个;住院神经症病人的噩梦中含较多家庭成员(男:h=-0.47,P=0.024;女:h=-0.80,P=0.000)、死亡与虚幻者(男:h=0.61,P=0.001;女:h=0.54,P=0.000),自我否定(男:h=0.74,P=0.000;女:h=0.56,P=0.000)、负性情绪(男:h=0.52,P=0.010;女:h=0.92,P=0.000)与不幸(男:h=0.65,P=0.003;女:h=0.61,P=0.000);其中女性神经症病人梦到较多的动物角色(h=0.54,P=0.000),含较多的攻击(h=0.63,P=0.000);相反,朋友(男:h=-0.47,P=0.024;女:h=-0.80,P=0.000)及成功(男:h=-0.80,P=0.000;女:h=-0.30,P=0.028)、友好(男:h=-0.45,P=0.038;女:h=-0.62,P=0.000)和性(男:h=-0.70,P=0.001;女:h=-0.38,P=0.005较少,噩梦场景多为室外不熟悉场景(男:h=-0.56,P=0.020;女:h=-0.74,P=0.000)。结论住院神经症病人噩梦中多出现死亡与虚幻者及动物,含较多攻击、自我否定、不幸与负性情绪体验,噩梦场景多为室外不熟悉场景。

Objective To study the characteristics of hospitalized neurosis patients＇ nightmare contents.Methods According to the psychiatric diagnosis standard CCMD-3 for neurosis,the hospitalized neurosis patients＇ nightmares were collected,then using the major categories of Hall and Van de Castle＇s system which was the nightmare content analysis to code each nightmare.At last,the nightmares will be analyzed with Dreams SAT.Results Totally 82 nightmares were collected;There were more family members（male：h=-0.47,P=0.024;female：h=-0.80,P=0.000）,Dead imaginary（male：h=0.61,P=0.001;female：h=0.54,P=0.000）,self-negativity（male：h=0.74,P=0.000;female：h=0.56,P=0.000）,negative emotions（male：h=0.52,P=0.010;female：h=0.92,P=0.000）and misfortune（male：h=0.65,P=0.003;female：h=0.61,P=0.000）,and more animal characters（h=0.54,P=0.000）,more aggression（h=0.63,P=0.000）in the hospitalized neurosis patients＇ nightmares;Instead,less friends（male：h=-0.47,P=0.024;female：h=-0.80,P=0.000）,success（male：h=-0.80,P=0.000;female：h=-0.30,P=0.028）,friendly（male：h=-0.45,P=0.038;female：h=-0.62,P=0.000） and sexual（male：h=-0.70,P=0.001;female：h=-0.38,P=0.005）were in them.nightmares scenes were more outdoor but not familiar with scenes（male：h=-0.56,P=0.020;female：h=-0.74,P=0.000）.Conclusion The hospitalized neurosis patients＇ nightmares appear more death imagination and animals,and contain more aggression character,self-negativity,misfortune and negative emotional experience,and nightmares＇ scene are more outdoor,and not familiar.
