二甲双胍联合缬沙坦治疗肥胖型高血压的疗效观察

Efficacy Observation of Metformin Combined with Valsartan in the Treatment of Obese Patients with Hypertension

目的:观察二甲双胍联合缬沙坦治疗肥胖型高血压的临床疗效。方法:选择我院2010年8月-2011年8月收治的106例肥胖型轻、中度高血压患者,随机将患者均分为治疗组和对照组,治疗组给予二甲双胍联合缬沙坦治疗,对照组仅给予二甲双胍治疗。比较2组患者服药前及服药3个月后,血压、甘油三酯(TG)、总胆固醇(TC)、空腹血糖(FBG)、糖化血红蛋白(HbA1C)以及体重指数(BMI)、腰围(WC)等指标的变化以及降压效果。结果:治疗3个月后,治疗组的降压效果明显优于对照组,差异有统计学意义(P<0.05)。治疗组各项指标均较治疗前有所降低,且TG、FBG、HbA1C、BMI水平与治疗前比较,差异有统计学意义(P<0.05);对照组仅HbA1C、BMI水平显著低于治疗前(P<0.05)。治疗后,2组HbA1C、BMI水平比较差异有统计学意义(P<0.05),其余各项指标无显著性差异(P>0.05)。试验过程中未见与本试验用药相关的不良反应。结论:二甲双胍联合缬沙坦治疗肥胖型高血压疗效显著,且安全性较好。

OBJECTIVE： To investigate the clinical efficacy of metformin combined with valsartan in the treatment of obese hy- pertensive patients. METHODS： 106 obese patients with mild to moderate hypertensive collected from our hospital during Aug. 2010- Aug. 2011 were randomized into treatment group and control group. Treatment group received metformin and valsartan, while control group only received metformin. The changes of blood pressure （systolic/diastolic） （SBP/DBP）, triglycerides （TG）, total cholesterol （TC）, fasting plasma glucose （FBG）, glycated hemoglobin （HbAlC） and body mass index （BMI）, waist circum- ference （WC） and other indicators were compared between two groups before medication and three months after medication as well as the antihypertensive effect. RESULTS： After 3 months of treatment, the antihypertensive effect of the treatment group was signif- icantly better than that of control group, the difference was statistically significant （P〈0.05）. In treatment group, all parameters were decreased after treatment, and the difference in the levels of TG, FBG, HbAlC and BMI had statistically significance （P〈 0.05） ; the levels of HbAlC and BMI in control group after treatment were significantly lower than before treatment（P〈0.05）. Af- ter treatment, the difference of HbAlC and BMI between 2 groups had statistically significance （P〈0.05）. Other indicators had no statistical difference（P〉0.05）. No drug-related adverse reaction was found during the test. CONCLUSION： Metformin combined with valsartan in the treatment of obese patients with hypertension have good effect and safety.
