不同证型初治肺结核患者T淋巴细胞亚群之间的关系

Relationship among Different Types of T-Lymphocyte Subsets of Patients with Initial Treatment of Pulmonary Tuberculosis

目的:观察不同证型初治肺结核患者T淋巴细胞亚群之间的关系,探讨其免疫状态改变的临床意义。方法:临床筛选120例初治活动性肺结核患者,中医证型分别符合肺阴亏虚型、阴虚火旺型、气阴两虚型的主症和次症。结果:气阴两虚组患者CD3+、CD4+均低于肺阴亏虚和阴虚火旺组,差异有统计学意义(P<0.01),阴虚火旺组CD3+、CD4+低于肺阴亏虚组,差异无统计学意义(P>0.05),其中以CD4+T淋巴细胞减少为主。结论:机体针对结核杆菌的免疫是T细胞介导的细胞免疫,以CD4+T淋巴细胞为主,有随疾病严重程度加重而降低的趋势。

Objective： To observe the relationship among the T-lymphocyte subsets of different types of patients with initial treatment of pulmonary tuberculosis;to explore the clinical significance of the immune status changes.Methods：120 cases of initial treatment of pulmonary tuberculosis were screened,the syndrome types should respectively meet the primary and secondary symptoms of Yin deficiency type,Hyperactivity of fire due to Yin deficiency type and Qi and Yin deficiency type.Results：CD＋3 and CD＋4 in the Qi and Yin deficiency type group were lower than those in the Yin deficiency type and Hyperactivity of fire due to yin deficiency type group,the difference was statistically significant（P0.01）.CD＋3 and CD＋4 in the Hyperactivity of fire due to yin deficiency type group were lower than those in the Yin deficiency type group,the difference had no statistical significance（P0.05）.The CD＋4 T-lymphocytes decrease was the main decrease.Conclusions：The body＇s immune against Mycobacterium tuberculosis is a T-cell mediated immunity,which mainly depends on CD＋4 T-lymphocytes,and has a trend to be severe with disease exacerbations.
