摘要
目的探讨葡萄糖耐量试验(OGTT)早期诊断高脂血症患者代谢综合征的临床意义。方法 120例单纯高脂血症患者行葡萄糖耐量试验后测定2 h血糖(2hPG),分析葡萄糖耐量降低(IGT)及代谢综合征检出情况,并比较正常组、IGT组及代谢综合征组患者空腹血糖(FPG)、2hPG、总胆固醇(TC)、甘油三酯(TG)及体重指数(BMI)。结果 120例患者经OGTT检出IGT 46例(38.3%),代谢综合征32例(26.6%)。三组FPG均在正常范围内,且差异无统计学意义(P>0.05)。IGT组及代谢综合征组患者2hPG较正常组显著升高(P<0.05、P<0.01)。三组TG、TC均高于正常值,代谢综合征组TG、TC较正常组显著升高(P<0.05),而IGT组TG、TC与正常组差异无统计学意义(P>0.05)。而代谢综合征组BMI较正常组显著增加(P<0.05),IGT组与正常组差异无统计学意义(P>0.05)。结论单纯高脂血症患者应及早行OGTT试验和BMI检查,以期及早发现代谢综合征发病情况,早期干预,降低心血管疾病的发病和死亡风险。
Objective To explore the clinical value of oral glucose tolerance test (OGTI') in early diagnosis of metabolic syndrome among patients with hyperlipidemia. Methods One hundred and twenty patients with single hyperlipidemia under- went OGTT, and then 2h plasma glucose (2hPG) was determined. Impaired gluccse tolerance (IGT) and metabolic syndrome were detected and observed. The levels of fasting plasma glucose (FPG), 2hPG, total cholesterol (TC), triacylglycerol (TG) and body mass index (BMI) were also compared among normal group, IGT group and metabolic syndrome group. Results Among 120 patients, 46 (38.3%) IGT patients and 32 (26.6%) metabolic syndrome patients were diagnosed by OGTT. FPG levels of the three groups were within normal values and showed no statistically significant difference (P 〉0.05 ). 2hPG levels of IGT group and metabolic syndrome group were significantly higher than that of normal group (P〈0.05, P〈0.01). TG and TC levels of the three groups were higher than normal values. TG and TC levels of metabolic syndrome group were significantly higher than those of normal group ( P 〈 0.05), but no statistically significant difference was found in TG and TC levels between IGT group and normal group (P 〉0.05). BMI of metabolic syndrome group was significantly higher than that of normal group ( P〈 0.05), but no statistically significant difference was observed in BMI between IGT group and normal group (P 〉0.05). Conclusions Patients with single hyperlipidemia should early undergo OGTT and BMI testing for definitive diagnosis of metabolic syndrome so as to conduct early interventions and reduce the incidence and mortality of cardiovascular diseases.
