摘要
目的观察类风湿性关节炎患者尿微量白蛋白(MA)、α1微球蛋白(α1-MG)、转铁蛋白(TRU)和免疫球蛋白G(IgU)的表达差异,比较应用尿微量蛋白与用尿常规和eGFR联合检测两种方法评价RA患者肾脏功能异常的差异性。方法应用免疫比浊法检测60例类风湿性关节炎患者和30例健康体检人员尿液MA、α1-MG、TRU、(IgU)、尿蛋白和血肌酐的含量。用SPSS11.5软件对结果进行统计分析。结果 RA患者肾功能异常组IgU、α1-MG与对照组比较差异有非常显著性(P<0.01),TRU、MA与对照组比较差异有显著性(P<0.05);RA患者肾功能正常组α1-MG与对照组比较差异有显著性(P<0.05),与RA患者肾功能异常组比较有差异非常显著性(P<0.01)。RA患者肾功能正常组TRU、MA与RA患者肾功能异常组比较差异有显著性(P<0.05);应用尿α1-MG判断RA患者肾功能异常与用尿常规和eGFR联合判断肾功能异常比较差异有非常显著性(P<0.01),应用尿MA、TRU、IgU判断RA患者肾功能异常与用尿常规和eGFR联合判断肾功能异常比较差异无显著性(P>0.05)。结论对类风湿性关节炎患者进行尿液微量蛋白的检测,有利于早期发现肾脏损伤,优于尿常规尿蛋白和eGFR联合监测。
Objective To observe the expression of miroproteins,MA,α1-MG,TRU and IgU,in random fresh urine sample of patients with RA and compare the difference of two methods for evaluating kidney function of patients with RA.Methods The level of microproteins was detected in random fresh urine of 60 RA and 30 healthy controls by immunonephelometry.Results were analyzed with SPSS software.Results IgU,α1-MG,TRU and MA in group with abnormal kidney function of RA are higher than that of control(P〈0.01-0.05).α1-MG in group with normal kidney function of RA is higher than that of control(P〈0.05),and lower than that of abnormal kidney function of RA(P〈0.01).TRU and MA are lower than that of abnormal kidney function of RA(P〈0.05).The sensitivity of determination of kidney function with α1-MG is higher than that with union of urine protein and eGFR(P〈0.05).Conclusion It is helpful to early find kidney injury for patients with RA detecting microproteins in their urine samples.It is better than union monitor of urine protein and eGFR.
