Study on Application Effect of Clinical Pathways for Community-acquired Pneumonia in Basic–level Hospitals

目的通过对基层医院社区获得性肺炎患者临床路径实施前后资料进行回顾性分析,评价临床路径应用于基层医院临床工作的实际效果。方法选择实施临床路径管理前后两个年度住我院内一科诊断为社区获得性肺炎患者为分析对象,将住院天数、住院费用、药占比、治疗效果、患者满意度等五个指标作为评价指标,进行对比分析。结果实施了临床路径的患者与未实行这一方法的患者之间在住院时间、住院费用、药占比、治愈率及患者满意度方面均有显著性差异(P<0.05)。结论实施社区获得性肺炎临床路径后,可以明显规范社区获得性肺炎的诊治,既可保证疗效,又可降低住院时间、住院费用及药占比,且患者对医疗服务满意度明显提高,值得在基层医院推广应用。

Objective To assess the actual effect of clinical pathways for community-acquired pneumonia through a retrospective analysis of data from basic- level hospitals. Methods Data before and after the implementation of clinical pathways for community-acquired pneumonia were compared and analyzed, including days of hospitalization, costs of hospitalization, the ratio of drugs, cure rate and Patient＇s satisfaction. Results A significant difference was observed in two groups. After the implementation of clinical pathways, days of hospitalization, costs of hospitalization and the ratio of drugs were reduced, while the cure rate and Patient＇s satisfaction were increased significantly. Conclusion After the implementation of clinical pathways for community-acquired pneumonia, the diagnosis and treatment process tends to be more standard, the cure rate and the satisfaction degree of patients on medical services were increased, while the costs of money and time in hodpital were reduced. So it is worth to promote the application in the basic hospital.
