经腹及经阴道超声诊断子宫肌瘤的临床比较

Abdominal and Transvaginal Ultrasound Diagnosis Uterine Fibroids Clinical Comparison

目的比较子宫肌瘤行经阴道及经腹超声检测结果，探讨阴道超声诊断价值。方法随机选择2010年1月至2011年12月经病理检查的95例子宫肌瘤患者作为研究对象，采用飞利浦11型彩色多普勒超声诊断仪，经阴道探头频率5-7MHz，经腹部探头频率3．5MHz；患者先经腹进行超声检查，然后排空膀胱行经阴道超声检查。结果肌壁间肌瘤70例；浆膜下肌瘤20例；黏膜下肌瘤5例，子宫肌瘤超声图像表现子宫增大者为特征，子宫轮廓规则增大者58例，不规则增大者47例，瘤体形态均呈圆形或椭圆形，有假包膜，边界尚清晰，内部回声多不规则。低回声型46例，强回声型14例，混合型35例。经阴道超声诊断率符合率97．89％（93／95）高于经腹超声的诊断符合率86．31％（P〈0．05）。结论阴道超声对诊断子宫肌瘤比腹部超声更具备优越性，具有安全、快速、无痛苦、可反复多次进行检查及价格低廉、患者易接受等优势，可作为可靠的首选方法。

Results Intramural fibroids 70 cases. Subserosal fibroids 20 cases. Submucous myoma of 5 cases, uterine fibroids ultrasound images show the uterus increases for the characteristics, the contour increased 58 cases of the rules, the irregular increase 47 cases were, tumors are a round or oval shape, false coated, boundary is still clear, internal echoes many irregular. Low echo 46 cases, strong echo type 14 cases, mixed type 35 cases, By transvaginal ultrasonography diagnosis rate were 97.89% was higher than the abdominal ultrasound diagnosis was 86.31% （P〈0.05）. Conclusion Vaginal ultrasound diagnosis uterine fibroids abdominal ultrasound than have more of the advantages, have safety, quick, no pain, can be repeated several times for inspection and low cost, easy to accept patients advantages and can be used as the first choice of reliable method.
