Application of Single-voxel and Multi-voxel MR Spectroscopy in the Detection of the Substantia Nigra in the Parkinson's Disease

摘要 目的探讨单体素和多体素1H-MRS在帕金森病(PD)患者黑质区检测中的优缺点。方法对20例PD患者与10例年龄匹配的健康对照者的黑质区进行单体素和多体素MRS,测定N-乙酰天门冬氨酸(NAA)、胆碱复合物(Cho)及肌酸复合物(Cr)含量,比较NAA/Cr、Cho/Cr、NAA/Cho比值以及比较单体素和多体素1H-MRS的谱线质量。结果多体素1H-MRSPD组NAA/Cr、NAA/Cho值减低,与对照组比较差异有统计学意义(P<0.05),多体素1H-MRS谱线质量稳定,能更准确全面地反映黑质代谢物的变化;单体素1H-MRS谱线质量差,无法用于诊断。结论多体素1H-MRS对PD患者黑质区进行检测,可以获得优质的谱线并为PD患者的病理生理改变和诊断提供重要信息。 Objective To evaluate the merits and demerits of single-voxel and multi-voxel MR Spectroscopy in the detection of the substantia nigra in patients with the Parkinson disease.Methods The content of NAA,Cho and Cr in substantia nigra in 20 patients and 10 age-matched healthy controls were examined with single-voxel and multi-voxel 1H-MRS.The rations of NAA/Cr,Cho/Cr and NAA/Cho were calculated.The quality of spectral line of single-voxel and multi-voxel 1H-MRS were compared.Results The ratios of NAA/Cr,NAA/Cho in PD patients with multi-voxel 1H-MRS showed significant reduction compared to controls(P0.05),multi-voxel 1H-MRS spectral lines of good quality can show the metabolic changes precisely in the substantia nigra in patients with PD;single-voxel 1H-MRS spectral lines quality is poor,can not be used for disgnosis.Conclusion Multivoxel 1H-MRS can get high quality of spectral lines and provide important information to understand the pathophysiological process of PD and diagnose PD by substantial nigra of PD patients.
