腹腔镜卵巢囊肿剔除术围术期卵巢功能及基质血流变化

目的 探讨腹腔镜卵巢囊肿剔除术围术期卵巢功能及卵巢基质血流的变化。方法 选取2009年10月～2011年6月于本院进行腹腔镜卵巢囊肿剔除术的45例患者为观察组,同时选取同期的45例进行开腹卵巢囊肿剔除术的患者为对照组,将两组患者术前及术后3个月、6个月的血清LH、FSH、E2、Fn及卵巢基质PI、RI、S/D、PSV进行检测及比较。结果 术后3个月、6个月观察组血清LH[(19.57±1.48)、(20.80±1.78)U/L]、FSH[(18.87±1.40)、(20.46±1.32)U/L]、E2[(257.36±10.12)、(249.63±9.84)pmol/ml]及Fn[(8.14±1.52)、(8.56±1.48)个]波动幅度小于对照组,而卵巢基质PI[(0.98±0.16)、(0.86±0.12)]、RI[(0.58±0.07)、(0.52±0.04)]、S/D[(2.63±0.22)、(2.48±0.18)]、PSV[(10.56±1.43)、(11.25±1.27)cm/s]改善幅度则大于对照组,两组上述指标比较,P均<0.05,差异有统计学意义。结论 腹腔镜卵巢囊肿剔除术对卵巢功能的影响较小,而在卵巢基质血流改善方面的优势也很明显,肯定了其临床疗效。

Objective To study the variation of ovarian function and ovarian stromal blood flow of patients treated with laparoscopic ovarian cyst resection during perioperative period. Methods 45 patients treated with laparoscopic ovarian cyst resection in our hospital from October 2009 to June 2011 were selected as observation group, and 45 patients treated with open ovarian cyst resection at the same time were selected as control group, then the serum LH, FSH, E2, Fn and ovarian stromal PI, RI, S/D, PSV of two groups before and after the surgery at third and sixth month were detected and compared. Results The fluctuation range of serum LH[（19.57±1.48, （20.80±1.78）U/L], FSH[（18.87±l.40）, （20.46±1.32）U/L], E2[（257.36±10.12）, （249.63±9.84）prnol/ml] and Fn[（8.14±1.52）, （8.56±1.48）]of observation group after the surgery at third and sixth month were all smaller than those of control group, the improvement range of ovarian stromal PI[（0.98±0.16）, （0.86±0.12）], RI[（0.58±0.07）, （0.52±0.04）], S/ D[（2.63±0.22）, （2.48±0.18）], PSV[（10.56±l.43）, （11.25±h27）cm/s] were all bigger than those of control group, these items of two groups were compared, all P〈0.05, there were all significant differences. Conclusion The influence of laparoscopic ovarian cyst resection for ovarian function is smaller, and the advantage of improving ovarian stromal blood flow is obvious, and these affirm its clinical effect.
