腹腔镜与开腹手术治疗异位妊的娠临床对比研究

The Clinical Comparative Study of Ectopic Pregnancy with Laparoscopic Surgery and Open Surgery

目的总结并探讨腹腔镜与开腹手术治疗异位妊娠临床效果差异及优缺点。方法选取我院2009年3月至2011年5月收治异位妊娠患者142例,随机分为两组,其中对照组71例,采用开腹手术治疗,实验组71例,采用腹腔镜手术治疗;治疗结束后评价临床疗效、不良反应发生情况及治疗费用等。结果两组患者均顺利完成手术,治疗成功率均为100%,且无膀胱或直肠损害,同时实验组中无中转开腹患者。实验组患者手术时间、术中出血量、术后肛门排气时间、术后下床时间、术后抗生素使用时间、术后镇痛时间及住院时间均明显少于对照组,组间比较差异有统计学意义(P<0.05);但是实验组患者治疗费用明显高于对照组,组间较差异亦有统计学意义(P<0.05)。结论与开腹手术相比较,腹腔镜治疗异位妊娠手术创伤小、术后恢复快,且并发症少,具有临床推广使用价值。

Objective To summarize and investigate the differences of therapeutic effect of ectopic pregnancy with laparoscopic surgery and open surgery and their advantages and disadvantages.Methods 142 patients with ectopic pregnancy were chose in the period from March 2009 to May 2011 in our hospital and randomly divided into two groups including control group（71 patients） with open surgery and experimental group（71 patients） with laparoscopic surgery and after treatment the therapeutic effects,adverse effects and treatment costs were assessed.Results All patients were successfully operated,and the treatment success rates were 100%.There was no damage to bladder or rectum of all patients,while none of experimental groups transferred to open surgery in the period of operation.The operation time,blood loss during operation,postoperative anal exhaust time,out-of-bed time after operation,antibiotics using time after operation,postoperative pain time,and staying time in hospital of experimental group were significantly less than control group（P0.05）.But the operation costs of experimental group were significantly more than control group（P0.05）.Conclusion Compared with open surgery,the ectopic pregnancy with laparoscopic surgery obtain less surgical trauma,faster postoperative recovery,have few adverse effects,and is worthy of clinical application.
