参附注射液对宫外孕手术患者血流动力学的影响

Impact of Shenfu Injecta on Patients’Hemodynamics with Ectopic Pregnancy Operation

目的探讨参附注射液对宫外孕手术患者血流动力学的影响。方法选取2009年6月至2011年10月在我院妇产科收治拟诊为宫外孕破裂出血需手术治疗患者共83例,随机分为观察组52例,对照组31例,对照组麻醉前乳酸林格氏液500mL静脉滴注,观察组给予酸林格氏液400mL加参附注射液100mL静脉滴注。观察两组患者术前出血量、术中出血量、手术时间、术前血红蛋白;心电监护麻醉前、气管插管时、气管插管后30min时患者心率、收缩压、舒张压、血氧饱和度的变化。结果①两组患者术前血红蛋白含量、术前出血量、术中出血量、手术时间比较无显著性差异(P>0.05);术中输液量观察组明显少于对照组存在显著性差异(P<0.05)。②两组患者HR插管时、麻醉后30min与麻醉前比较下降,SBP、DBP升高,存在显著性差异(P<0.05);观察组插管时、麻醉后30min HR、SBP、DBP明显高于对照组,存在显著性差异(P<0.05);两组SpO2组间及组内比较均无显著性差异(P>0.05)。结论参附注射液对宫外孕手术患者血流动力学变化有稳定作用。

Objective To investigate the impact of Shenfu Injecta on patients’ hemodynamics with ectopic pregnancy operation.Methods Selected 83 cases patients diagnosed as ruptured ectopic pregnancy in the gynaecology and obstetrics of our hospital from June 2009 to October 2011,randomly divided them into observation group（52 cases） and control group（31 cases）.The control group were given 500ml lactic acid forest Grignard solution intravenous infusion before anesthesia,the observation group were given 100ml acid Ringer＇s solution plus 400ml Shenful intravenous infusion.The preoperative blood loss,blood loss,operative time,preoperative hemoglobin,ECG monitoring before anesthesia,endotracheal intubation,patients at 30min after intubation heart rate,systolic and diastolic blood pressure,blood oxygen saturation degree of change of the two groups were observed.Results ①There was no significant difference between the two groups’preoperative hemoglobin,preoperative blood loss,blood loss,operative time（P0.05）;the intraoperative fluid volume of the observation group was obviously less than that of the control group,with a significant difference（P0.05）.②The HR when intubation and 30min after anesthesia of the two groups was lower than before the anesthesia,SBP,DBP increased,there was a significant difference（P0.05）;HR,SBP,DBP when intubation and 30min after anesthesia of the observation group were obviously higher than those of control group,with a significant difference（P0.05）;there was no significant difference between SpO2 of the two groups and within them（P0.05）.Conclusion Shenfu Injecta has a stabilizing effect on the patients’ hemodynamics with ectopic pregnancy operation.
