围手术期胰岛素泵治疗妊娠期糖尿病的临床分析 被引量:6

摘要 目的:探讨胰岛素泵对妊娠期糖尿病围手术期患者的临床疗效。方法:将80例妊娠期糖尿病患者随机分入对照组与观察组,其中38例对照组患者接受每日多次皮下注射胰岛素,42例患者胰岛素泵持续泵入胰岛素。比较两组治疗后血糖控制情况、血糖达标时间、胰岛素用量、低血糖发生率、切口愈合时间及术后并发症发生率。结果:两组患者血糖治疗后均显著下降,但观察组血糖达标时间、胰岛素用量、切口愈合时间均显著少于对照组(P<0.01);观察组与对照组低血糖发生率分别为2.4%和10.5%,术后并发症发生率分别为2.4%和13.2%,差异具有统计学意义(P<0.01)。结论:妊娠期糖尿病围手术期应用胰岛素泵模拟生理性胰岛素分泌模式,尽快达到靶血糖水平,降糖效果快速平稳,使妊娠期糖尿病患者安全度过围手术期。 Objective: To explore the clinical effect of insulin pump on gestational diabetes mellitus during perioperation period. Methods:A total of 80 cases with gestational diabetes mellitus were divided into control group and experiment group. A total of 38 cases in control group received multi subcutaneous insulin injection and 42 patients were treated with continuous subcutaneous insulin infusion by insulin pump. Blood sugar, time of reaching target of glucose, dosage of insulin, time of wound healing, accidence of hypoglycemia and postoperative morbidity rate were compared between two groups. Results: Blood sugar level greatly decreased after treatment in two groups(P〈0.01), but time of reaching target of glucose and time of wound healing were significantly shorter, and dosage of insulin was significantly lower in experiment group than those in control group(P〈0.01) ; Accidence of hypoglycemia was 2.4% vs. 10.5Mand postoperative morbidity rate were 2.4% vs. 13.2% in experiement group and control group with significantly difference (P〈0.01). Oonclusions:Insulin pump in perioperation period can simulate the physiologic insulin secretion, stably, and make patients safely restore after operations. gestational diabetes mellitus during reduce blood sugar more faster and stably, and make patients safely restore after operations.
