摘要 目的 研究阿尔茨海默病 (AD)神经心理学测验、脑 SPECT特征及与帕金森病 (PD)的辅助鉴别诊断价值。方法 对 30例正常老人 (NA) ,30例 AD和 31例 PD进行神经心理学测验和 SPECT检查 ,比较三组认知功能和脑血流灌注特点。结果 三组间各测验分数均有显著差异 ,NA各测验得分最高 ,AD各测验得分最低 ,AD和 PD认知功能损害模式不同 ;AD病人的脑血流灌注可能呈不对称下降 ,脑血流灌注下降最大的顶叶、右颞叶与 BD存在正相关。结论 神经心理学测验和脑SPECT可作为诊断和鉴别诊断 AD和 PD的辅助手段。 Objective To explore the characteristics of neuropsychological tests and SPECT with 99mTc HMPAO of Alzheimer disease(AD) and their differential diagnostic value between AD and Parkinson's disease(PD). Methods Five neuropsychological tests and SPECT were performed in 30 normal aged(NA), 30 AD and 31 PD to compare the difference on cognitive functions and cerebral perfusion. Results Cognitive functions assessed by neuropsychological tests were NA>PD>AD.The cognitive impairment modes were different between AD and PD. The perfusion of AD may decrease asymmetrically. There was correlation between cerebral perfusion and neuropsychological test scores. The perfusion in parietal and right temporal cortex was correlated with BD results. Conclusion Neuropsychological tests and brain SPECT are of help in the process of diagnosis and differentiation between AD and PD.
