改良结膜梯形切除联合结膜巩膜固定术治疗结膜松弛症

Improved trapezoid conjunctival resection and sclera fixation for conjunctivochalasis

目的观察改良结膜梯形切除联合结膜巩膜固定术治疗结膜松弛症的安全性和术后效果。方法按照诊断标准收集中、重度结膜松弛症患者31例62只眼,行改良的结膜梯形切除联合结膜巩膜固定术,观察术中、术后并发症及术后效果,并对患者的症状进行评估。结果术后临床治愈58只眼(93.55%);泪膜破裂时间≥10 s者57只眼(91.94%);泪河连续完整,泪河线宽度≥0.3 mm者56只眼(90.32%);术后自觉症状评估,治愈38只眼,改善19只眼,总有效率91.94%。术后3只眼出现球结膜下少许出血,术中、术后无其它并发症发生。结论改良的结膜梯形切除联合结膜巩膜固定术是治疗中、重度结膜松弛症的一种安全可靠、疗效满意的方法。

OBJECTIVE To evaluate the safety and postoperative effect through observation on improved trapezoid conjunctival resection and sclera fixation for conjunctivochalasis,and to discuss a new surgical method.METHODS Thirty-one（62 eyes） middle and severe conjunctivochalasis were collected according to the diagnosis criterion.They were operated by improved trapezoid conjunctival resection and sclera fixation,then observed the complications and curative effect during operation and postoperation,and evaluated the symptoms in patients.RESULTS Of 3 eyes appeared subconjunctival hemorrhage after operation without other complications during operation and postoperation.The clinical cure rate was 93.55%;BUT≥10 seconds in 57 eyes（91.94%）;The complete tears river and tears river line width ≥0.3mm in 56 eyes（90.32%）;The total effective rate of self-conscious symptoms of postoperation was 91.94%.CONCLUSIONS Improved trapezoid conjunctival resection and sclera fixation was a safe,effective and new method for middle and severe conjunctivochalasis.
