脂肪来源干细胞辅助脂肪移植的现状、不足与展望 被引量:2

摘要 脂肪移植在面部年轻化、软组织重建、乳腺再造、创伤修复等整形外科领域有着广泛的应用前景.但由于脂肪组织不耐缺氧、易发生液化及"瘤"变等自身特性,目前还不能进行广泛的临床应用[1],而脂肪来源干细胞(adipose-derived stem cells, ADSC)为这一领域带来新的希望.近年来,针对其特性开展的一系列基础研究及临床应用研究,虽已取得了很大进展,但也暴露出许多问题.干细胞辅助脂肪移植(cell-assisted lipotransfer, CAL)是组织工程学的概念,即在脂肪移植过程中加入ADSC,用来促进移植部位的血管再生及提高移植脂肪的成活率.Matsumoto等[2]研究表明,在ADSC辅助下,可以增加35%的移植脂肪成活率,特别是与透明质酸支架作为载体或与前脂肪细胞混合等新技术的结合[3],使CAL成为新的研究热点,并已取得许多重大的突破.笔者将从ADSC提取、鉴别、临床应用、远期疗效评价等方面具体分析其现状与不足,旨在综合分析CAL从基础研究到临床应用所亟待解决的问题,希望能对本领域的进一步发展有所助益.
