视网膜栓子与颈内动脉病变和血脂异常的相关性

Correlation of retinal embolism with severity of carotid artery stenosis and serum lipid abnormalities

目的分析视网膜栓子患者颈内动脉病变狭窄的程度和血脂异常等因素的相关性。方法本研究对2009年至2011年在我院眼科门诊进行眼底检查及眼底照相确诊有视网膜栓子的35例患者进行血脂分析和颈动脉多普勒检查。按照视网膜栓子的类型将其分为2组:Hollenhorst组和非Hollenhorst组。并对两组患者的合并症、血脂及颈动脉多普勒结果进行统计分析。结果 Hollenhorst组和非Hollenhorst组同侧颈动脉狭窄率分别为(30.5±14.5)%和(65.1±9.8)%,差异有显著统计学意义(P=0.000)。Hollenhorst组和非Hollenhorst组总胆固醇含量分别为(4.99±0.71)mmol·L-1和(4.45±0.64)mmol·L-1,差异有统计学意义(P=0.026),高密度脂蛋白含量分别为(1.18±0.34)mmol·L-1和(0.97±0.13)mmol·L-1,差异有统计学意义(P=0.036)。而2组甘油三酯、低密度脂蛋白含量比较,差异均无统计学意义(均为P>0.05)。结论眼底视网膜栓子的发现与颈动脉病变和血脂异常的程度具有相关性,详细的眼底检查对于患者全身疾病的治疗和预后具有一定的意义。

Objective To investigate the correlation of retinal embolism with severity of carotid artery stenosis and serum lipid abnormalities.Methods Thirty-five patients with retinal embolism in our hospital from 2009 to 2011 were diagnosed by digital fundus imaging.Lipid measurements and color Doppler ultrasound examinations of carotid artery were performed.According to the type of retinal embolism,patients were divided into two groups：Hollenhorst group（group 1） and non-Hollenhorst group（group 2）.The complications,serum lipid and ultrasound examinations results in two groups were statistically analyzed.Results The rate of ipsilateral carotid artery stenosis in Hollenhorst and non-Hollenhorst group were（30.5±14.5）% and（65.1±9.81）%,respectively,there was significant difference（P=0.000）;The content of total cholesterol were（4.99±0.71）mmolL-1 and（4.45±0.64） mmolL-1,respectively,there was statistical difference（P=0.026）;The content of high density lipoprotein were（1.18±0.34） mmolL-1 and（0.97±0.13） mmolL-1,respectively,there was statistical difference（P=0.036）;There was no statistical difference in triglyceride and low density lipoprotein（both P0.05）.Conclusion The appearance of retinal embolism is correlated with the severity of carotid artery stenosis and serum lipid abnormalities.Detailed retinal examination is necessary for the treatment and prognosis of the patients.
