血清同型半胱氨酸和胱抑素C水平与冠状动脉病变严重程度的相关性 被引量:4

摘要 目的分析血清同型半胱氨酸(HCY)和胱抑素C(Cys-C)与冠状动脉粥样硬化型心脏病(CHD)患者冠状动脉病变支数、稳定情况以及血清HCY与Cys-C的相关性。方法对128例CHD患者和50例健康人进行了血清HCY和Cys-C水平检测。结果 (1)不稳定型心绞痛(UAP)、稳定型心绞痛(SAP)及健康对照(NC)组之间血清HCY和Cys-C水平均差异均具有统计学意义(P<0.01);UAP、SAP组血清HCY和Cys-C水平均显著高于NC组(P<0.01);UAP组血清HCY和Cys-C水平均显著高于SAP组(P<0.01)。(2)CHD患者单支病变、双支病变及多支病变组血清HCY和Cys-C水平差异均具有统计学意义(P<0.01),且随病变支数的增加而升高,多支病变组均显著高于双支病变组(P<0.01),双支病变组显著高于单支病变组(P<0.05)。(3)CHD患者血清HCY与Cys-C呈正相关(r=0.830,P<0.01),病变支数与血清HCY和Cys-C呈正相关(r=0.702、0.581,P均小于0.01)。结论血清HCY和Cys-C水平与CHD冠状动脉病变、病变稳定性及病变支数有关,血清HCY与Cys-C也具有相关一致性,联合检测血清HCY和Cys-C水平,可更好地了解病情、指导治疗及判断预后。 Objective To investigate the correlation between homocycteine(HCY) and serum cystatin C (Cys- C) with the injuried coronary artery branch number, stability in the patients with coronary heart disease(CHD ) and correlation between serum HCY and Cys-C. Methods To analyze serum HCY and Cys-C levels,and injuried coro nary artery branch number,stability and correlation between serum HCY and Cys-C in 128 cases of CHD identified by coronary arteriography. Results (1) Serum HCY and Cys-C levels had statistical difference among the unstable angina pectoris(UAP) group,the stable angina pectoris(SAP) goup and the normal control(NC) group (P〈0.01). Serum HCY and Cys-C levels in the UAP group and the SAP group were significantly higher than those in the NC group(P〈0.01). Serum HCY and Cys-C levels in the UAP group were significantly higher than those in the SAP group(P〈0.01). (2) Serum HCY and Cys-C levels had statistical difference among the single branch lesion,double branch lesions and multiple branch lesions groups (P〈0. 01) ,and which were increased with the increase of branch lesions number, the multiple branch lesions group was significantly higher than the double branch lesions group (P〈0.01) ,the double branch lesions group was significantly higher than the single branch lesion group(P〈0.05/P〈 0.01). (3)Serum HCY and Cys-C showed significantly positive correlation in CHD (r=0. 830, P〈50.01 ), the branch lesions number was significantly correlated with serum HCY and Cys-C (r=0. 702,0. 581, P〈0.01). Conclusion Serum HCY and Cys C levels are related with coronary artery lesion,stability and branch lesion number in CHD. Ser um HCY and Cys-C have the related consistency. Combined detection of serum HCY and Cys-C can better understand the disease condition ,guide treatment and judge prognosis.
