二甲双胍联合克罗米芬治疗多囊卵巢综合征合并不孕的疗效分析

Analysis on curative effect of metformin combined with clomiphene in treatment of polycystic ovary syndrome combined with infertility

目的:探讨二甲双胍联合克罗米芬治疗多囊卵巢综合征合并不孕患者的疗效。方法:将收治的多囊卵巢综合征合并不孕患者96例随机分为观察组和对照组各48例,观察组接受二甲双胍联合克罗米芬治疗,对照组仅接受克罗米芬治疗。结果:观察组体质指数(BMI)、空腹胰岛素(FINS)、促黄体生成素(LH)、LH/促卵泡刺激素(LH/FSH)、睾酮(T)在治疗后显著降低,治疗前后比较差异有统计学意义(P<0.05);而FSH、雌二醇(E2)治疗前后相比差异无统计学意义(P>0.05)。对照组BMI、FINS、LH、LH/FSH、T、FSH、E2治疗前后相比差异无统计学意义(P>0.05)。治疗后观察组BMI、FINS、LH、LH/FSH、T显著低于对照组,差异有统计学意义(P<0.05);两组患者的FSH、E2相比差异无统计学意义(P>0.05)。治疗后观察组的排卵率、妊娠率显著高于对照组,差异有统计学意义(P<0.05)。结论:二甲双胍联合克罗米芬可提高多囊卵巢综合征患者对胰岛素的敏感性,并降低其LH、T等激素水平,提高患者的排卵率与妊娠率。

Objective： To explore the curative effect of metformin combined with clomiphene in treaiment of polycystic ovary syndrome （PCOS） combined with infertility. Methods： Ninety - six patients with PCOS combined with infertility were divide into observation group and control group, 48 patients in each group, the patients in observation group were treated with metformin combined with elomiphene, while the patients in control group were treated with clomiphene. Results： After treatment, body mass index （BMI）, fasting insulin, luteinizing hormone （ LH）, the ratio of LH to follicle stimulating hormone （ LH/FSH）, and testosterone （T） in observation group decreased signif- icantly, there was significant difference before and after treatment （P 〈 0. 05） ; but there was no significant difference in the levels of FSH and estradiol before and after treatment （ P 〉 0. 05 ） . In control group, there was no significant difference in BMI, fasting insulin, LH, LH/ FSH, T, FSH, and estradiol before and after treatment （P 〉0. 05） . After treatment, BMI, fasting insulin, LH, LH/FSH, and T in observation group were significantly lower than those in control group （ P 〈 0. 05 ） ; there was no significant difference in the levels of FSH and estradiol between the two groups （P 〉 O. 05） . After treatment, the ovulation rate and pregnancy rate in observation group were significantly higher than those in control group （P 〈 0. 05 ） . Conclusion： Metformin combined with clomiphene can improve the sensitivity of PCOS patients to insulin, reduce the levels of LH and T, and raise ovulation rate and pregnancy rate.
